Pigmentation, melasma, and dark spots

Pigment sits at different depths. Sharply bordered spots such as freckles and sun spots sit near the surface, while melasma is a broader, hazy patch in which pigment often sits at more than one depth.

Overview

Skin colour comes from melanin made by pigment cells in the lower epidermis. When those cells are triggered, by ultraviolet light, by heat, by inflammation after acne or a procedure, or by hormonal change, they produce more of it and the extra pigment settles unevenly. Where it settles decides how it looks and how it behaves.

Sharply bordered spots such as freckles and sun spots usually sit near the surface. Melasma is different: it appears as a broader patch with a hazy border, often across the cheeks, forehead, or upper lip, it is strongly driven by sun and heat, and pigment in it often sits at more than one depth. That is why melasma is described as managed rather than removed, and why the same brown patch can behave quite differently in two people who look the same in a photograph.

Concern FAQ

What if the physician recommends something different from what I asked for?

This happens, and it is worth understanding why. When a physician examines your anatomy in person, they often find that the treatment you arrived expecting is either less suited to your tissue than an alternative, or that the combination you read about online does not match your actual clinical picture.

The recommendation will be explained - not handed to you as a take-it-or-leave-it directive. You will understand the reasoning before anything is agreed. If you have a strong preference for a specific approach, say so; the physician will clarify what the clinical tradeoffs are.

Nothing is performed without your informed consent. If after the consultation you prefer to go elsewhere or think further before proceeding, that is a reasonable outcome.
